CareHarmony is looking for a Remote Intake Coordinator - Licensed Practical Nurse (LPN) to play a key role in delivering comprehensive, value-based care management. You will work directly with healthcare providers to support patients on their healthcare journey.
What You'll Do
- Work comprehensively with providers to deliver value-based care management initiatives for patients.
- Identify resources and coordinate needs for chronic care management patients.
What We're Looking For
- Active Licensed Practical Nurse (LPN), NLC, or LVN license.
- At least 3 years of direct patient-facing work experience.
Benefits & Compensation
- Compensation: $21/hr.
- Fully remote position.
- Consistent schedule - Full-Time Monday–Friday, no weekends.
- Rotational on-call once per year on average.
