Chronic Care Staffing is looking for a detail-oriented and patient-focused Remote CCM/RPM Enrollment Coordinator to support our chronic care management programs. In this role, you will be the initial point of contact, introducing patients to Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services, obtaining verbal consent, and serving as a trusted liaison between patients and their care teams.
What You'll Do
- Conduct outreach to patients for CCM and RPM enrollment.
- Assess patient eligibility and explain program benefits clearly and professionally.
- Obtain and document verbal consent in full compliance with CMS guidelines.
- Educate patients and families on chronic conditions, medications, and care expectations.
- Communicate effectively with healthcare providers and internal care teams.
- Promote positive patient experiences through proactive and compassionate outreach.
- Support care plan adherence and encourage patient self-management strategies.
- Ensure all documentation meets CMS, HIPAA, and CCS compliance standards.
What We're Looking For
- Certification as a Medical Assistant (CMA or RMA).
Benefits & Compensation
- Hourly pay: $17.00 – $20.00.
