Position Overview
This remote role supports newly enrolled individuals by guiding their care journey through assessment, planning, and ongoing coordination. Based in Fort Lauderdale, FL, the Case Manager ensures members receive timely, person-centered services while working within a structured interdisciplinary framework.
Key Responsibilities
- Conduct screenings and comprehensive assessments to understand member needs
- Establish measurable goals and support members in building self-management skills
- Create, execute, and track personalized service plans aligned with individual preferences
- Coordinate care transitions and monitor reassessment timelines based on risk levels
- Act as the primary contact for members, maintaining consistent communication
- Facilitate collaboration among providers, caregivers, social workers, and care coordinators
- Ensure all documentation is accurate, secure, and compliant with regulatory standards
- Compile and summarize data for performance tracking and reporting
- Support quality improvement initiatives and adhere to organizational protocols
- Manage a dynamic caseload while prioritizing member safety and service access
Qualifications
Candidates must hold one of the following credentials: Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Social Worker (LCSW), or Registered Nurse (RN). An RN must have a minimum of a Bachelor’s degree; all other applicants must possess a Master’s degree. The ideal candidate will be located in Fort Lauderdale, FL (33304), available during standard business hours (8 AM – 5 PM), and capable of fulfilling all clinical and administrative duties remotely.
Work Environment
This is a hybrid remote position requiring residency in the specified location. The role operates on a temp-to-perm basis, offering the potential for long-term engagement based on performance and organizational needs.
