Tangelo is seeking a Registered Dietitian with an active New York State RD credential to support programs across New York in a remote, contract capacity. In this role, you will be integral to our holistic model of care, assessing members' nutritional needs, prescribing medically tailored meals, and providing culturally relevant education and counseling.
What You'll Do
- Report to the Senior Director, Clinical Operations.
- Participate in developing nutrition standards and health education for chronic conditions using the latest clinical evidence.
- Provide individualized, evidence-based health education and counseling to members.
- Create tailored nutritional interventions and ensure food prescriptions are properly transmitted to the fulfillment team.
- Deliver comprehensive nutrition services, including screening, assessment, counseling, and education.
- Account for religious, cultural, and ethical factors in all assessments and interventions.
- Evaluate potential food, drug, and herb/supplement interactions during education and counseling.
- Collaborate with members to set achievable goals and create personalized care plans.
- Support social care navigation and enrollment processes to improve member outcomes.
- Maintain active credentials and stay current with practices in clinical nutrition and dietetics.
What We're Looking For
- Bachelor’s degree in Nutrition, Dietetics, or a related field from an accredited institution.
- Must hold an active RD credential and be licensed to practice in New York State.
- Fluency in Mandarin or Cantonese is required.
- Strong understanding of clinical nutrition, particularly related to chronic conditions and medically tailored interventions.
- Experience working with electronic health records (EHRs) and providing care through telehealth platforms.
- Training or experience in motivational interviewing, especially for social care coordination or patient-centered care.
- Detail-oriented with the ability to assess complex dietary needs in a systems-based, operational setting.
- Professional, compassionate communicator with a collaborative, team-first mindset.
