Chronic Care Staffing is seeking a Registered Dietitian licensed in South Carolina to join our Chronic Care Management team. This fully remote role focuses on providing dietary counseling and virtual support to patients managing chronic conditions and weight loss goals through Remote Patient Monitoring programs.
What You'll Do
- Track and interpret patient weight readings and other relevant health data for remote patient monitoring.
- Conduct engaging one-on-one nutrition coaching sessions via telehealth.
- Develop individualized meal plans, including for patients on medications such as Ozempic.
- Provide guidance on nutrition, healthy eating habits, and lifestyle changes.
- Partner with physicians, pharmacists, and healthcare teams for coordinated care.
- Maintain accurate records in EMR and remote monitoring platforms.
What We're Looking For
- Active Registered Dietitian (RD) license in South Carolina.
- Experience in chronic conditions, weight loss, or related fields.
- Tech-savvy with experience using remote monitoring platforms and/or EMR systems.
- Excellent communication skills with the ability to engage patients remotely.
- Must live/work full-time in the United States (any EST or CST state).
Nice to Have
- Experience in the Medicare CCM (Chronic Care Management) program.
- Background in RPM (Remote Patient Monitoring).
