Theoria Medical seeks a Physician Assistant or Nurse Practitioner to deliver high-quality primary care to patients residing in skilled nursing facilities. You will ensure comprehensive clinical management through daily rounding and meticulous documentation.
What You'll Do
- Round and manage the clinical care of the population at your assigned Skilled Nursing Facility (SNF).
- Prescribe medications and treatments for residents.
- Provide high quality, patient-centered, comprehensive primary care services.
- Complete timely documentation of all patient care encounters.
- Be available for phone and secure messaging consultations with facility nursing staff.
- Complete documentation in Theoria’s proprietary electronic health record system, ChartEasy™.
What We're Looking For
- Valid PA or NP license in the state of practice.
- Valid NP or PA degree from an accredited institution.
- Active DEA license (Theoria will assist new graduates with application).
- Familiarity with EMR systems and charting practices.
