Touchstone Health was looking for a Physical Therapist

Touchstone Health is looking for a PRN Physical Therapist to join our team serving patients in the Kerrville and Bandera area. In this role, you will help patients regain mobility, confidence, and independence directly in their homes, offering flexibility and the opportunity to build meaningful relationships with a diverse patient population.

What You'll Do

  • Conduct evaluations, reassessments, and recertifications in alignment with Medicare, state licensure, and Touchstone Health standards.
  • Develop and update individualized plans of care based on clinical judgment and patient needs.
  • Deliver skilled interventions, home exercise programs, and education to promote independence, mobility, and safety.
  • Document care accurately and timely, including OASIS assessments, progress notes, and ICD-10 coding requirements.
  • Collaborate with physicians, PTAs, Home Health Aides, and interdisciplinary team members to ensure aligned care planning.
  • Provide clear instruction and coaching to patients and caregivers, adapting communication to each unique home setting.
  • Supervise PTAs in compliance with state practice acts and organizational policies.
  • Participate in QAPI initiatives and staff education to support continuous improvement.

What We're Looking For

  • Licensed Physical Therapist (PT) in the state of practice.
  • Graduate of an accredited physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation.
  • Current CPR certification.
  • Ability to exercise sound clinical judgment and work independently in varied patient care settings.
  • Strong communication and interpersonal skills with sensitivity to diverse backgrounds.
  • Basic computer proficiency; willingness to learn EMR systems and OASIS documentation.
  • Commitment to compliance with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies.
  • Reliable transportation, valid driver’s license, and auto insurance for home visits.
  • Must be able to stand, walk, bend, and assist with mobility or equipment as needed. Requires the ability to perform heavy lifting and to travel locally in various weather conditions.

Nice to Have

  • Two years of clinical experience preferred.
  • Home health experience a plus but not required.

Work Mode
