CVS Health is hiring a Pediatric Case Manager - Registered Nurse to join our team. In this full-time, remote telehealth role, you will be responsible for assessing, planning, and coordinating care for disengaged, vulnerable members with multiple medical, behavioral, and social challenges, with a focus on facilitating wellness and positive health outcomes.
What You'll Do
- Assess, plan, implement, and coordinate all case management activities with members to evaluate medical needs and facilitate overall wellness.
- Develop proactive courses of action to address issues and enhance short and long-term outcomes.
- Use clinical tools, data review, and clinical judgment to conduct evaluations and incorporate risk-reduction strategies.
- Conduct holistic assessments considering co-morbid and multiple diagnoses impacting functionality.
- Collaborate with supervisors and key stakeholders in the member's healthcare to overcome barriers.
- Utilize case management processes in compliance with regulatory and company policies.
- Apply motivational interviewing skills to ensure maximum member engagement.
- Work on behalf of the client and the member's provider.
What We're Looking For
- Registered Nurse residing in the Continental United States.
- Unrestricted nursing license with multi-state/compact privileges and ability to be licensed in all non-compact states.
- 3+ years of clinical/nursing practice experience.
- 3+ years of specific pediatric experience.
- 1+ year(s) of experience in care coordination or discharge planning.
- High-speed internet with ability to hard wire computer.
- 3+ years of experience with personal computers, keyboarding, multi-systems navigation, and MS Office Suite applications.
- Associate's degree in nursing.
Nice to Have
- Telephonic case management experience.
- Excellent communication and interpersonal skills.
- BSN.
