Braze is looking for a Patient Engagement Specialist to connect patients with critical health and social services through compassionate, high-volume outreach. In this remote role, you’ll play a key part in a mission-driven organization revolutionizing mental healthcare by building trust, scheduling appointments, and ensuring seamless handoffs to clinical teams for individuals with complex needs.
What You'll Do
- Conduct high-volume phone and virtual outreach to educate patients and caregivers about health services and available benefits.
- Help patients and caregivers schedule and attend recommended healthcare appointments.
- Independently organize and prioritize your workload to meet monthly outreach and enrollment goals.
- Utilize our CRM and electronic medical record system to track outreach and document member consent and interaction notes.
- Work collaboratively with care teams to ensure a seamless transition from outreach to ongoing support.
- Provide feedback on outreach strategies to improve our patient-focused approach.
- Participate in virtual meetings and maintain motivation in a fully remote, dynamic work environment.
What We're Looking For
- 2+ years of experience in customer service or sales.
- 2+ years of experience in healthcare outreach, community engagement, or a related field.
- Strong ability to initiate conversations and build connections quickly, especially with individuals with vulnerable health care needs.
- Experience in healthcare, social services, or mental health services, with an understanding of seniors’ needs.
- Proficiency in critical thinking, time management, and multitasking.
- Able to work independently and meet weekly outreach goals successfully.
- Basic proficiency in G-Suite and video conferencing platforms.
- Adherence to HIPAA regulations and confidentiality.
- Strong tech proficiency and comfort using technology in daily work.
