Mount Carmel Health System is seeking a Patient Care Pharmacist to join our team. This role operates on a hybrid night shift schedule, working 7 consecutive days followed by 7 consecutive days off, with 50% remote work, blending in-person patient care with remote pharmacy duties.
What You'll Do
- Provide comprehensive pharmaceutical care to patients, ensuring safe and effective medication use.
- Review medication orders for appropriateness and collaborate with healthcare providers on therapy plans.
- Perform clinical tasks remotely as part of the hybrid work model during night shifts.
- Counsel patients on medication regimens and monitor for therapeutic outcomes.
- Maintain accurate patient records and ensure compliance with all regulatory standards.
What We're Looking For
- Doctor of Pharmacy (Pharm.D.) degree or equivalent from an accredited institution.
- Valid and active pharmacist license in the state of Ohio.
- Ability and willingness to work the designated hybrid night shift schedule (7 days on, 7 days off).
- Strong clinical judgment and excellent attention to detail.
- Effective communication skills for interacting with patients, families, and the healthcare team.
Work Mode
This is a hybrid position with a 50% remote work component. You will follow a 7-days-on, 7-days-off night shift schedule, requiring flexibility and the ability to work effectively both on-site and from a remote location.
