The Patient Care Coordinator plays a central role in guiding patients through their care journey within a virtual-first specialty clinic. As a primary point of contact, you’ll deliver compassionate support while managing communication, documentation, and coordination across clinical teams and external providers.
Key Responsibilities
- Serve as the main liaison for patient inquiries via secure messaging, phone, and EHR systems
- Coordinate appointments, prescription renewals, diagnostic testing, imaging, and referrals
- Collaborate with clinicians and outside providers to ensure timely prior authorizations and care continuity
- Document patient encounters accurately and maintain up-to-date EMR records
- Monitor patient progress and proactively identify delays or access barriers
- Support onboarding for newly enrolled patients with clear, empathetic guidance
- Contribute to process improvements by identifying inefficiencies and developing automation solutions
- Participate in team meetings and help refine workflows to enhance care delivery
- Respond promptly to inbound patient messages and ensure follow-through on care tasks
Qualifications
Candidates must have prior experience in patient-facing healthcare roles and be comfortable working within EST hours. Proficiency with electronic medical records, HIPAA-compliant documentation, and telehealth platforms is essential. Strong organizational skills, clear communication, and the ability to manage complex care coordination are required.
Preferred candidates will have experience supporting patients with rare or complex conditions such as POTS, EDS, or MCAS, and familiarity with virtual care models. Experience with automation tools or process optimization in healthcare settings is a plus.
