Responsibilities
- Perform comprehensive health and medication assessments with patients, including verifying medication lists and administration details.
- Accurately input prescription data to ensure correct medication fulfillment and delivery.
- Meet or exceed established performance benchmarks defined by leadership.
- Foster a positive initial experience that reflects the organization’s service excellence.
- Inform patients, facilities, and institutions about care offerings and processes.
- Oversee patient evaluations to support business growth and sustain referral partnerships.
- Keep detailed records and generate reports to ensure consistent communication across teams.
- Contact potential patients regularly to schedule telehealth reviews and conduct follow-up calls.
- Attend daily team check-ins, training sessions, and occasional meetings as required.
- Maintain open communication channels with supervisors and management.
- Demonstrate core organizational values in every professional interaction.
- Complete additional tasks as assigned by leadership.
