What You'll Do
- Provide comprehensive occupational therapy assessments and develop individualized treatment plans for home health clients
- Implement therapeutic interventions to improve clients' abilities to perform activities of daily living
- Educate patients and their families on treatment plans, adaptive techniques, and safety measures
- Maintain accurate and timely documentation of patient evaluations, progress notes, and discharge summaries
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care
What We're Looking For
- Valid California state license as an Occupational Therapist
- Graduate of an accredited occupational therapy program
- Experience in home health, rehabilitation, or a related clinical setting
- Strong clinical assessment, critical thinking, and patient education skills
- Excellent written and verbal communication abilities
- Valid driver's license and reliable transportation for travel within the Oakland area
Work Mode
