Pinnacle Home Care is looking for an Occupational Therapist to deliver compassionate healthcare in patients' homes. In this role, you will assess patient functional status, design and implement treatment programs, and collaborate closely with physicians and healthcare teams to promote patient independence and well-being.
What You'll Do
- Assess patient functional status, including muscle function, endurance, self-care, and work capacity.
- Evaluate the home environment for hazards or barriers and identify necessary adaptive equipment.
- Collaborate in developing and communicating comprehensive, goal-oriented care plans.
- Fabricate splints and instruct patients on the use of adaptive, prosthetic, and orthotic devices.
- Teach or retrain patients in daily living activities lost due to illness or injury.
- Maintain accurate clinical records and report changes in patient condition to physicians.
- Participate in team meetings, case conferences, and in-services to promote coordinated care.
- Provide health education to patients and their families or caregivers.
- Identify needs for additional services and make appropriate referrals.
- Supervise Certified Occupational Therapy Assistants per agency and state regulations.
What We're Looking For
- Current state-specific Occupational Therapist license.
- Graduate of an accredited occupational therapy program recognized by the AMA and AOTA.
- Registered with the National Registration Examination of the AOTA.
- Valid BLS certification.
- Minimum of two years of occupational therapy experience.
- Excellent communication, organizational, and time management skills.
- Valid driver’s license and access to a reliable, insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care.
- Ability to engage in physical activities including walking, standing, stooping, and occasional patient handling.

Work Mode
This role provides care in patient homes across Florida. You will be traveling locally to deliver services.
