DaVita is looking for a full-time Nurse Practitioner to support patients with complex kidney disease, delivering holistic and integrated care to improve their quality of life. You will coordinate and facilitate quality, consistent, and cost-effective care, working both autonomously and collaboratively.

What You'll Do

Complete Comprehensive Health Assessments.
Manage complex patients with a focus on reducing hospitalizations and improving health outcomes.
Work primarily from your home office and travel within an assigned geography, including to dialysis clinics and nephrology practices.
Maintain a flexible Monday-Friday schedule to accommodate patient and partner availability.

What We're Looking For

Passion for delivering care to patients with complex, chronic illnesses.
Minimum of two (2) years of experience as a Nurse Practitioner (NP).
Current unrestricted NP license to practice in the state of New York.
Ability to get licensed in other states, primarily PA, OH, and WV as needed.
Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
Current CPR or Basic Life Support (BLS) certification.
Intermediate computer skills and proficiency in MS Word, Excel, and Outlook.
Home office setup, high-speed internet, and reliable transportation are required.

Nice to Have

Experience in Value Based Care or Population Health, including documenting and coding from a Medicare Advantage risk perspective and conducting Health Risk Assessments.
5+ years of experience as a Family Nurse Practitioner, Primary Care NP, or experience in Internal Medicine, Cardiology, Nephrology, or Case Management.
Experience managing chronic diseases such as CHF, diabetes, and hypertension.
Experience managing population health to reduce hospitalizations.

Compensation: $115,000.00 - $140,000.00 per year.
