DaVita is looking for a full-time Nurse Practitioner to support patients with complex kidney disease, delivering holistic and integrated care to improve their quality of life. You will coordinate and facilitate quality, consistent, and cost-effective care, working both autonomously and collaboratively.
What You'll Do
- Complete Comprehensive Health Assessments.
- Manage complex patients with a focus on reducing hospitalizations and improving health outcomes.
- Work primarily from your home office and travel within an assigned geography, including to dialysis clinics and nephrology practices.
- Maintain a flexible Monday-Friday schedule to accommodate patient and partner availability.
What We're Looking For
- Passion for delivering care to patients with complex, chronic illnesses.
- Minimum of two (2) years of experience as a Nurse Practitioner (NP).
- Current unrestricted NP license to practice in the state of New York.
- Ability to get licensed in other states, primarily PA, OH, and WV as needed.
- Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
- Current CPR or Basic Life Support (BLS) certification.
- Intermediate computer skills and proficiency in MS Word, Excel, and Outlook.
- Home office setup, high-speed internet, and reliable transportation are required.
Nice to Have
- Experience in Value Based Care or Population Health, including documenting and coding from a Medicare Advantage risk perspective and conducting Health Risk Assessments.
- 5+ years of experience as a Family Nurse Practitioner, Primary Care NP, or experience in Internal Medicine, Cardiology, Nephrology, or Case Management.
- Experience managing chronic diseases such as CHF, diabetes, and hypertension.
- Experience managing population health to reduce hospitalizations.

- Compensation: $115,000.00 - $140,000.00 per year.

Work Mode
This is a hybrid position based in the Buffalo, NY area, requiring you to live locally. You will work from your home office with travel as needed within an assigned geography.
