CareHarmony is seeking a Nurse Practitioner to deliver high-quality, patient-centered primary care services to residents at assigned skilled nursing facilities (SNFs). You'll join a supportive community focused on meaningful connections and appreciation.
What You'll Do
- Round daily and manage the clinical care of the population at assigned Skilled Nursing Facilities (SNFs)
- Prescribe medications and treatments for residents
- Provide comprehensive primary care services with a patient-centered approach
- Complete timely documentation of all patient encounters
- Be available for phone and secure messaging consultations with facility nursing staff
- Complete documentation in our proprietary EHR system, ChartEasy™
- Work with facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech
- Be on-site and on time for all scheduled in-person shifts
- Maintain timely attendance and have reliable transportation
What We're Looking For
- Master’s or Doctorate degree in Nursing from an accredited program
- Active and unrestricted RN and NP license in the state of practice
- National board certification as an NP from AANP or ANCC
- DEA License required or ability to obtain
- Current BLS/ACLS certification as required by the facility
- Strong clinical assessment and decision-making skills
- Excellent communication and documentation skills
- Ability to work independently and collaboratively within an interdisciplinary team
