About the Role
This position involves delivering comprehensive primary care through virtual visits, managing both chronic and acute conditions, and supporting patient wellness using a technology-enabled care model tailored to the needs of Illinois residents.
Responsibilities
- Provide virtual primary care consultations for patients across Illinois
- Diagnose and treat acute medical conditions via telehealth
- Manage chronic illnesses including diabetes, hypertension, and asthma
- Conduct comprehensive patient history assessments remotely
- Order and interpret diagnostic tests as clinically appropriate
- Develop individualized treatment plans based on patient needs
- Prescribe medications following state and federal regulations
- Maintain accurate and timely electronic medical records
- Ensure continuity of care through follow-up visits
- Collaborate with care coordination teams to support patient outcomes
- Educate patients on health maintenance and disease prevention
- Adhere to clinical protocols and quality standards
- Respond to patient messages within established timeframes
- Participate in quality improvement initiatives
- Support patient engagement through clear communication
- Uphold patient confidentiality and HIPAA compliance
- Stay current with medical guidelines and best practices
- Assist in onboarding and mentoring new providers as needed
