Theoria Medical is seeking a Manager of Chronic Care Management (CCM) to oversee day-to-day operations and clinical staff for our CCM program. In this remote, full-time role, you will develop the program to ensure optimal patient outcomes.
What You'll Do
- Oversee and manage the day-to-day operations of the CCM program.
- Supervise and mentor clinical staff, including pharmacists, nurses, and other healthcare providers.
- Develop and implement strategies to improve the CCM program.
- Coordinate with interdisciplinary teams on patient care.
- Participate in tech development projects to streamline CCM processes.
- Ensure adherence to regulatory guidelines and maintain up-to-date knowledge of developments in the field.
- Analyze patient data to guide improvements in the CCM program.
What We're Looking For
- Bachelor's degree in Healthcare Administration, Public Health, Nursing, or a related field.
- Experience overseeing a chronic care or similar program.
- Strong leadership and management skills.
- Excellent interpersonal and communication skills.
- Strong computer and technical skills.
- Understanding of the role of a CCM program in healthcare delivery.
Nice to Have
- Master’s degree.
- Experience with telehealth platforms.
