Grüns is looking for a Licensed Practical Nurse to join our team focused on remote patient monitoring and hospital avoidance. In this role, you will be instrumental in proactively managing patient health through technology and coordinated care.
What You'll Do
- Proactively manage patient health by monitoring remote patient monitoring dashboards and responding to vital sign alerts.
- Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage.
- Coordinate timely follow-up care with Nurse Practitioners, Primary Care Providers, or patient caregivers.
- Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems.
- Ensure patient compliance and smooth operation by troubleshooting remote monitoring device issues.
- Act as the primary clinical point of contact for an assigned panel of remote care patients.
- Assist with patient enrollment and onboarding activities as needed to support growth.
- Collaborate daily with the clinical team, intake staff, and support personnel.
What We're Looking For
- An active LPN license in Virginia.
- Strong clinical judgment combined with excellent verbal communication and phone skills.
- Highly organized, proficient with technology, and adept at managing simultaneous digital platforms.
- Ability to thrive in a structured, fast-paced in-office team environment.
- A primary internet connection with a minimum speed of 15 Mbps.
- A backup internet connection with at least 10 Mbps capable of supporting work.
- A desktop or laptop with an Intel Core i5 (8th gen+), Intel Core i3 (10th gen+), AMD Ryzen 5, or equivalent processor.
- A desktop or laptop with a minimum of 8 GB RAM.
Nice to Have
- Prior experience in Remote Patient Monitoring, telehealth, or complex care coordination.
- A deep commitment to preventive care and reducing hospital readmissions and ER utilization.
