DaVita, whose name means 'giving life,' is hiring a Licensed Practical Nurse (LPN) to provide essential, life-saving care to dialysis patients in an outpatient clinical setting. This role is about building long-term relationships with patients and their families while working collaboratively in a fast-paced, supportive team environment.

What You'll Do

Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
Deliver care to patients who often have multiple co-morbidities, requiring unique treatment plans and a broad range of nursing skills.
Work with your head, heart, and hands each day in direct patient care.
Float to various facilities during and after your training.
Commit to improving patient health through clinical goal setting and quality improvement initiatives.

What We're Looking For

Current LPN-LVN license in the state of practice is required.
Current CPR certification is required.
IV certification is required if mandated by state regulations.
High school diploma or equivalent is required.
Completion of a training program approved by the medical director and governing body, as required by CMS guidelines, prior to operating the water treatment system.
Basic computer skills and proficiency in MS Word and Outlook are required.
Functional proficiency in DaVita's computer systems within 90 days of hire is required.
A flexible schedule and ability to work mornings, evenings, weekends, and holidays.

Nice to Have

BONENT or CHT certification is preferred or as required by state regulation.
A minimum of one year of experience in a hospital or clinical setting is preferred.
