Forbes Advisor is looking for an Intake Physical Therapist to join our team. In this role, you will be responsible for conducting initial patient evaluations and establishing treatment plans.
What You'll Do
- Perform comprehensive initial physical therapy assessments for new patients.
- Develop and document individualized care plans based on patient evaluations.
- Coordinate with referring physicians and healthcare providers to ensure continuity of care.
- Educate patients and their families on treatment goals and expected outcomes.
What We're Looking For
- Valid state license to practice as a Physical Therapist.
- Proven experience in patient intake, evaluation, and care plan development.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal abilities.
