CVS Health is seeking a Nurse Practitioner or Physician Assistant to join our team on a part-time basis in Rochester, NY and the surrounding area. This role with Signify Health, part of CVS Health, involves traveling to members' homes to perform comprehensive health evaluations, build trusted relationships, and help bridge gaps in care.
What You'll Do
- Provide In-Home Health Evaluations, engaging with people in their homes to help bridge gaps in care.
- Conduct visits lasting about 45 minutes, including medication and medical history review and a physical evaluation.
- Perform additional diagnostic tests if ordered by the health plan, such as diabetic eye exams or spirometry.
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Be flexible to travel locally and within licensed states, as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
What We're Looking For
- Active, unrestricted license(s) in your coverage area(s).
- Board certification.

- Able to cover the following primary locations: Rochester and surrounding area.
