Touchstone Health is hiring a Home Health Physical Therapist Assistant to administer physician-prescribed physical therapy on an intermittent basis in patients' homes. You'll work under the direction and supervision of a Registered Physical Therapist in accordance with established plans of care.
What You'll Do
- Understand and adhere to established Agency policies and procedures.
- Provide physician prescribed physical therapy under a plan of care established by the PT.
- Improve or minimize residual physical disabilities of the patient.
- Return the individual to optimum and productive level within the patient’s capabilities.
- Participate with all other health care personnel in patient care planning.
- Perform all skilled procedures as ordered by physician and according to the plan of care.
- Consult with PT regarding changes in treatment.
- Instruct patients and family members in home programs and activities of daily living.
- Participate in and present in-service programs as assigned.
- Participate in QAPI activities as assigned.
- Attend all patient care conferences as scheduled.
- Prepare medical records, progress notes, and update care plans for each patient visit in a timely manner.
- Perform services planned, delegated, and supervised by the PT.
- Assist in preparing clinical and progress notes.
- Participate in educating the patient and family.
- Perform maintenance therapy services under a program established by a qualified therapist.
What We're Looking For
- Graduate from a two-year college level program approved by the American Physical Therapy Association.
- Currently licensed, registered, or certified in the state(s) in which practicing.
- Acceptance of philosophy and goals of Agency.
- Ability to exercise initiative and independent judgment.
Nice to Have
- Two (2) years’ experience.
