Halcyon Home LLC is hiring a Home Health Physical Therapist Assistant for a PRN Pay Per Visit position. You will support our growing geriatric home health population by providing physical therapy services to patients in their homes across the Waco/Temple, TX area and surrounding communities.
What You'll Do
- Perform ongoing clinical assessments, including OASIS, to identify patient needs and determine functional levels.
- Develop and evaluate the plan of care in partnership with the patient and their caregivers.
- Document clinical notes, phone orders, and other required patient information promptly.
- Provide services ordered by the physician as detailed in the established plan of care.
- Apply infection control concepts and standard precautions to prevent disease transmission.
- Accept clinical assignments that match your education and professional competence.
- Provide supervision and guidance to physical therapy assistants and home health aides, as needed.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
- Communicate with all physicians and other healthcare practitioners involved in the patient's care.
- Meet mandatory continuing education requirements to maintain licensure and skills.
- Assist and instruct patients and families in transfer techniques and the proper use of equipment.
- Provide counseling and education to patients, caregivers, and family members.
What We're Looking For
- Current and valid PTA license
Nice to Have
- Minimum of one year of home health experience
- Knowledge or experience utilizing Homecare Homebase EMR
- Experience in a home health or related healthcare setting
- Strong clinical and patient assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
