Halcyon Home LLC is seeking a dedicated and compassionate Home Health Physical Therapist (PRN) to join our team serving a growing geriatric population in Waco, TX and the surrounding area. You will provide essential in-home physical therapy, building relationships with patients and families while collaborating closely with a supportive care team.
What You'll Do
- Perform ongoing clinical assessments, including OASIS, to identify patient needs and determine levels of functioning.
- Develop and evaluate personalized plans of care in partnership with patients, their representatives, and caregivers.
- Document, prepare, and complete all clinical notes, phone orders, and required record documentation on an ongoing basis.
- Provide services ordered by the physician as indicated in each plan of care.
- Apply concepts of infection control and standard precautions consistently.
- Accept clinical assignments that align with your education and professional competence.
- Provide supervision and guidance to physical therapy assistants and home health aides as appropriate.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
- Communicate effectively with all physicians and other health care practitioners involved in patient care.
- Meet all mandatory continuing education requirements.
- Assist and instruct patients and families in transfer techniques and the use of equipment such as walkers, wheelchairs, and canes.
- Provide patient, caregiver, and family counseling and education.
What We're Looking For
- A current, valid Physical Therapist (PT) license.
Nice to Have
- Knowledge or experience utilizing Homecare Homebase EMR.
- Experience in a home health or related healthcare setting.
- A minimum of one year of direct home health experience.
