Halcyon Home LLC is hiring a Home Health Physical Therapist Assistant to deliver physical therapy to geriatric patients in community and residential settings within the South Austin, TX area. You will join a team dedicated to patient-centered care.
What You'll Do
- Perform ongoing clinical assessment, including OASIS assessments, to identify patient needs and determine level of functioning.
- Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
- Document, prepare, and complete clinical notes, phone orders, and other clinical record documentation on an ongoing basis.
- Provide services ordered by the physician as indicated in the plan of care; oversee other team members in patient care.
- Apply concepts of infection control and standard precautions to prevent contamination and transmission of disease.
- Accept clinical assignments consistent with education and competence.
- Provide supervision and guidance to physical therapy assistants and home health aides, as appropriate.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
- Communicate with all physicians involved in the plan of care and other health care practitioners.
- Meet mandatory continuing education requirements.
- Assist and instruct patients and families in transfer techniques and use of equipment (walkers, wheelchair, canes).
- Provide patient, caregiver, and family counseling and education.
What We're Looking For
- Current PTA license
Nice to Have
- Minimum of one year of home health experience
- Knowledge or experience utilizing Homecare Homebase software
- Experience in home health or related healthcare setting
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
- Proven ability to manage a fast-paced, independent work schedule
