About the Role

Halcyon Home LLC is hiring a Home Health Physical Therapist Assistant to deliver physical therapy to geriatric patients in community and residential settings within the South Austin, TX area. You will join a team dedicated to patient-centered care.

What You'll Do

  • Perform ongoing clinical assessment, including OASIS assessments, to identify patient needs and determine level of functioning.
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
  • Document, prepare, and complete clinical notes, phone orders, and other clinical record documentation on an ongoing basis.
  • Provide services ordered by the physician as indicated in the plan of care; oversee other team members in patient care.
  • Apply concepts of infection control and standard precautions to prevent contamination and transmission of disease.
  • Accept clinical assignments consistent with education and competence.
  • Provide supervision and guidance to physical therapy assistants and home health aides, as appropriate.
  • Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
  • Communicate with all physicians involved in the plan of care and other health care practitioners.
  • Meet mandatory continuing education requirements.
  • Assist and instruct patients and families in transfer techniques and use of equipment (walkers, wheelchair, canes).
  • Provide patient, caregiver, and family counseling and education.

What We're Looking For

  • Current PTA license

Nice to Have

  • Minimum of one year of home health experience
  • Knowledge or experience utilizing Homecare Homebase software
  • Experience in home health or related healthcare setting
  • Strong clinical and assessment skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and manage time effectively
  • Proven ability to manage a fast-paced, independent work schedule
