Halcyon Home LLC is hiring a Home Health Physical Therapist Assistant (PRN) to support our growing geriatric home health population. You will provide essential physical therapy services to patients in community and residential settings across South Austin, TX and the North Austin area.
What You'll Do
- Perform ongoing clinical assessment, including OASIS assessments, to identify patient needs.
- Develop and evaluate the plan of care in partnership with patients, representatives, and caregivers.
- Document, prepare, and complete clinical notes, phone orders, and other required records.
- Provide services ordered by the physician as indicated in each patient's plan of care.
- Apply concepts of infection control and standard precautions.
- Accept clinical assignments consistent with your education and competence.
- Provide supervision and guidance to physical therapy assistants and home health aides as needed.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
- Communicate with all physicians involved in the plan of care and other healthcare practitioners.
- Meet mandatory continuing education requirements.
- Assist and instruct patients and families in transfer techniques and safe equipment use.
- Provide patient, caregiver, and family counseling and education.
What We're Looking For
- Current PTA license.
Nice to Have
- Knowledge or experience utilizing Homecare Homebase EMR.
- Experience in a home health or related healthcare setting.
- A minimum of one year of home health experience.
