About the Role
The role is responsible for aligning organizational performance with value-based care objectives, overseeing the development and execution of strategies that enhance patient outcomes while reducing costs. The leader will work closely with clinical and operational teams to track performance metrics and lead continuous improvement initiatives.
Responsibilities
- Lead the design and implementation of value-based care strategies
- Collaborate with clinical leadership to improve patient outcomes
- Develop performance dashboards to track quality and cost metrics
- Identify opportunities for operational efficiency within care delivery
- Guide teams through transitions to value-based payment models
- Ensure compliance with regulatory and payer-specific value requirements
- Analyze clinical and financial data to inform strategic decisions
- Facilitate cross-departmental alignment on value initiatives
- Oversee reporting to external quality and performance programs
- Partner with finance to model financial impact of care changes
- Lead process improvement projects focused on cost and quality
- Translate policy changes into actionable operational plans
- Support care coordination and population health efforts
- Engage providers in adoption of evidence-based practices
- Monitor benchmarks and performance targets across programs
- Drive accountability for performance at the department level
- Evaluate new value-based contracts and participation opportunities
- Work with legal and compliance on risk-sharing arrangements
- Develop training materials on value-based care principles
- Present findings and recommendations to executive leadership
- Foster a culture of data-driven decision-making
- Manage external consultants and vendor partners as needed
- Ensure patient experience metrics are integrated into value goals
- Align incentive structures with value outcomes
- Maintain up-to-date knowledge of industry trends in value-based care
