About the Role

Sharecare is hiring a Forms Completion Specialist to manage FMLA and Disability forms with precision and efficiency. This remote role is central to our mission of delivering supportive, high-quality customer service to patients and their providers.

What You'll Do

  • Process FMLA and Short Term Disability paperwork in a timely manner.
  • Communicate with patients and physician coordinators using a positive, patient-centered approach.
  • Process medical record requests with strict adherence to HIPAA and HITECH laws.
  • Complete accurate forms by applying industry-standard responses based on the specialty practice.
  • Answer incoming calls across multiple lines, log data, and provide respectful, attentive service.
  • Validate and process all incoming requests for Protected Health Information (PHI).
  • Ensure patient disability forms are completed within a 5-7 day turnaround after payment.
  • Pull patient forms and PHI requests daily for invoicing and system entry.
  • Establish and maintain professional relationships with all account clients.
  • Monitor Electronic Medical Record (EMR) accounts to ensure requests are processed within required timeframes.
  • Verify patient information using key identifiers.
  • Conduct quality screenings on incoming PHI to safeguard patient data.
  • Verify requesting party contact information, including fax numbers and addresses.
  • Update and maintain an Accounting of Disclosure Log for all released medical records.
  • Provide careful attention and care to patients and their representatives.

What We're Looking For

  • 1+ year of prior experience in a medical records department or similar setting.
  • Strong computer skills with general working knowledge of Microsoft Word and Excel.
  • Excellent organizational skills and the proven ability to multitask.
  • A typing speed of at least 50 words per minute.
  • Ability to use standard office equipment like fax machines, copiers, and scanners.
  • Willingness to learn new equipment and processes quickly.
  • Self-motivated, team-oriented nature with proven customer satisfaction skills.
  • Excellent verbal and written communication skills.
  • Familiarity with medical terminology and standard medical office processes.
  • Required experience with FMLA/Disability forms.
