Aledade seeks a Director, Community Health Center Performance to drive strategic initiatives and performance optimization for our partner community health centers. In this remote role, you will be instrumental in enhancing the effectiveness of value-based care models and supporting health centers in delivering better patient outcomes.
What You'll Do
- Develop and implement performance improvement strategies for community health centers within Aledade's network.
- Analyze clinical and operational data to identify opportunities for enhancing care quality and financial performance.
- Lead cross-functional projects to support health centers in transitioning to and succeeding in value-based care arrangements.
- Build and maintain strong collaborative relationships with health center leadership and clinical teams.
- Create and deliver reports and presentations on center performance, trends, and recommendations.
What We're Looking For
- 7+ years of experience in healthcare performance management, value-based care, or a related field.
- Proven track record of leading successful initiatives within community health centers or similar primary care settings.
- Deep understanding of value-based care models, quality metrics, and healthcare reimbursement.
- Strong analytical skills with experience using data to inform strategy and drive operational change.
- Excellent communication and stakeholder management skills.
Nice to Have
- Direct experience working in or with Federally Qualified Health Centers (FQHCs).
- Familiarity with healthcare data platforms and analytics tools.
