FOODMatch is seeking a Community Health Worker to join the Maryland Primary Care Program (MDPCP AHEAD). In this role, you will provide short-term care coordination and connect patients to vital community and state resources, working directly with primary care providers and social services agencies across Maryland.
What You'll Do
- Coordinate care among individuals, communities, and health and social service systems.
- Conduct outreach through telephonic, office, and home visits.
- Provide culturally appropriate health and informational resources.
- Oversee case management and resource access.
- Advocate for individuals and communities regarding housing, food security, and other social determinants of health needs.
- Provide coaching and social support in areas like nutrition, recovery services, and chronic disease management.
- Organize and/or provide services such as basic health screenings.
- Increase community participation in health assessments and education.
What We're Looking For
- Must be located in the DMV (DC, Maryland, Virginia) area, with Maryland preferred.
Team & Environment
You will interact and collaborate with interdisciplinary care teams, population health teams, networked facility partners, physicians, and others to deliver patient-centered care.
Work Mode
This position operates on a hybrid schedule within the DMV (DC, Maryland, Virginia) area, focusing on the Maryland market.
