Rockefeller Philanthropy Advisors is hiring a Clinical Program Manager for its Interpersonal Violence Reduction Program. This role leads a remote team of Care Extenders who support members' holistic well-being, combining strategic program management with direct, hands-on care coordination across multiple states.
What You'll Do
- Lead a remote team of Care Extenders, providing coaching, supervision, and support to ensure quality service delivery and member outcomes.
- Act as a player/coach by actively engaging in care activities, particularly in challenging situations, while modeling best practices for team members.
- Oversee service planning, member assessments, care coordination, and progress monitoring to ensure whole-person care.
- Support team members in crisis intervention, risk evaluation, and safety planning for at-risk members.
- Promote member empowerment by encouraging skill development, social engagement, and goal achievement.
- Ensure team members consistently engage with assigned members and meet program KPIs.
- Coordinate caseloads and support referrals, scheduling, and operational planning for care teams.
- Participate in recruiting, onboarding, and performance management of team members.
- Lead regular internal case reviews, participate in external meetings with health partners, and ensure thorough documentation.
- Analyze team performance data to identify gaps and design improvement strategies.
- Collaborate with cross-functional teams on billing accuracy, data reporting, and administrative responsibilities.
What We're Looking For
- Active licensure as a Qualified Mental Health Professional (QMHP) in one of the following: LCSW, LPC/CPC, LMFT, Licensed Psychologist (PhD/PsyD).
- License must be active or eligible for transfer to one of these states: Indiana, Louisiana, Nevada, Tennessee, Texas, or Florida.
- Master’s degree in behavioral health, social work, or healthcare management from an accredited university.
- Proven leadership experience in clinical program management, including team supervision and outcome delivery.
- Strong understanding of care coordination, case management, and community-based interventions.
- Excellent communication, problem-solving, and data analysis skills.
Nice to Have
- Preferred experience in health coaching or medical social work.
