myPlace Health is seeking a Clinical Pharmacist to join our team on-site at our clinic in Greater Los Angeles. In this role, you will be integral to our patient-centered care model, working directly within the clinic to support our community.
What You'll Do
- Provide direct, comprehensive medication management and clinical pharmacy services to patients.
- Collaborate with physicians, nurses, and other healthcare providers to optimize medication therapy.
- Conduct patient consultations and education to improve adherence and health outcomes.
- Participate in quality improvement initiatives and medication safety programs.
What We're Looking For
- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
- Active, unrestricted pharmacist license in the state of California.
- Completion of a PGY1 residency or equivalent clinical pharmacy experience in an ambulatory care setting.
- Strong interpersonal and communication skills for effective team collaboration and patient interaction.
Work Mode
This is an onsite position based at our clinic in Los Angeles, CA. You will be expected to work on-site to provide direct patient care and team collaboration.
