Sentara Health is hiring a Clinical Liaison to play a critical role in connecting patients with post-acute rehabilitation services. You will be responsible for obtaining referrals and facilitating the patient admission process through thorough clinical assessment, benefit verification, patient and family education, and interdisciplinary collaboration.
What You'll Do
- Obtain referrals and initiate clinical review to ensure medical necessity.
- Conduct a thorough assessment of all clinical documentation and patient/caregiver interviews to determine admission eligibility.
- Educate potential patients, families, and the community on the benefits of our post-acute rehabilitation services.
- Establish and maintain networking connections with referral sources through ongoing communication and education.
- Verify patient insurance, identify plan benefits, and obtain pre-authorization if required.
- Complete the insurance process and all appropriate documentation within the Electronic Medical Record.
- Ensure all regulatory requirements for admission are met prior to facility transfer.
- Efficiently coordinate all patient transfer activities in collaboration with case management, physicians, and nurses.
- Manage bed assignments.
- Contribute to the collection and analysis of key performance indicators related to admissions.
What We're Looking For
- A Master’s Degree or Bachelor's in Physical Therapy or a Master’s Degree in Occupational Therapy or a Master’s Degree in Speech Therapy or an Associate’s Degree as a Registered Nurse.
- 3 years of healthcare-related clinical experience.
- Excellent problem-solving skills.
- Ability to comprehensively assess a patient's appropriateness for post-acute rehabilitation.
- Strong clinical data analytic skills for measuring process and outcomes in healthcare.
- Strong interdisciplinary communication skills.
- American Heart Association Basic Life Support certification for Pennsylvania upon hire.
- A Pennsylvania state license upon hire as a Registered Nurse (RN) or Licensed Physical Therapist (PT) or Licensed Occupational Therapist (OT) or Licensed Speech Language Pathologist (SLP).
Nice to Have
- A Doctorate in Physical Therapy/Occupational Therapy or a Master’s of Business/Master’s of Healthcare Administration or a Master’s of Nursing.
- 1 year of Inpatient Rehabilitation experience and 1 year of Clinical Liaison experience.
- EMR experience.
- Insurance authorization experience.
Work Mode
This is a remote position open to candidates located in Pennsylvania.
