CVS Health is seeking a Clinical Case Manager, Behavioral Health DSNP to remotely facilitate physical and behavioral healthcare for members. You will be a critical part of our mission to build a more connected, convenient, and compassionate world of health, working with a team passionate about transforming care.
What You'll Do
- Utilize advanced clinical judgment and critical thinking skills to facilitate appropriate member healthcare through assessment and care planning.
- Coordinate directly with providers and coordinate psychosocial wraparound services to promote effective resource utilization and optimal, cost-effective outcomes.
- Conduct comprehensive assessments of referred member's needs and eligibility using clinical tools and information/data review.
- Determine the approach to case resolution by evaluating the member's benefit plan and available internal and external programs/services.
- Apply clinical judgment to incorporate strategies designed to reduce risk factors and address complex clinical indicators impacting care planning.
- Perform crisis intervention with members experiencing a behavioral health or medical crisis and refer them to appropriate clinical providers.
- Provide crisis follow-up to members to help ensure they are receiving appropriate treatment and services.
What We're Looking For
- Master's Degree in Social Work or Counseling required.
- Valid, unrestricted independent professional behavioral health clinical license to practice per state regulations in your state of residence (e.g., LCSW, LISW, LCPC, LP, LMFT, LMHC or equivalent).
- 3+ years of direct clinical practice experience post-master's degree (e.g., hospital setting, ambulatory care, or outpatient clinic/facility).
- 3+ years of knowledge of mental health and substance abuse disorders.
- 3+ years of experience using Motivational Interviewing techniques.
- 3+ years of experience talking on the telephone and typing simultaneously.
- Good technological skills.
Nice to Have
- Crisis intervention skills.
- Managed care or utilization review experience.
- Case management and discharge planning experience.
