Clarvida is looking for a part-time CBRS/Case Manager to deliver community-based behavioral health services to clients ages 5 through adult. You will empower clients by building skills, accessing community resources, and providing coping strategies to foster independence and improve quality of life.
What You'll Do
- Deliver CBRS and Case Management services to clients at their home and in the community, focusing on individual behavioral and/or mental health needs.
- Work with clients ages 5 through adult who have behavioral issues, emotional disturbance, or severe and persistent mental health diagnoses.
- Maintain documentation throughout the case including daily progress notes, treatment plan updates, and discharge summaries.
- Meet with clients weekly to focus on skill building, gaining access to community resources, and providing coping skills.
What We're Looking For
- Bachelor’s degree in a Human Service field (Social Work, Psychology, Sociology, Human Service, Education, Counseling, Family Relations, Nursing, Physical Therapy, Occupational Therapy, Therapeutic Recreation, Speech Language Pathology, Behavioral Sciences, Art Therapy, etc.)
- Ability to pass background checks, including Enhanced Fingerprinting with ID DHW.
- Valid driver’s license with reliable transportation and proof of car insurance.
