40Position Summary
We're looking for a compassionate care coordinator who will be the critical link between patients, their families, and comprehensive healthcare services.Primary Duties & Responsibilities:
Conduct thorough, sensitive assessments with clients and their support networks to develop personalized care plans that address individual needs.
Strategically arrange and coordinate service referrals, connecting clients/families with appropriate resources and public benefits programs as they qualify.
Maintain meticulous documentation of all client contacts and service coordination activities, ensuring compliance with program guidelines.
Provide targeted client/family education about medical and psychosocial aspects of health conditions to enhance treatment adherence and understanding.
Execute proactive follow-up by consistently tracking symptoms and monitoring treatment responses.
Strictly adhere to all state regulations and departmental rules governing healthcare programs.
Remain current with emerging resources and developments through continuous training, staff meetings, and community engagement.

$52,600.00 - $78,900.00 / AnnuallyThe salary range reflects base salaries paid for positions in a given job grade across the University.
