Vaughan Regional Medical Center / ScionHealth is hiring a Case Manager II to coordinate and facilitate patient care through effective collaboration with the Interdisciplinary Care Transitions team. You will follow patients throughout the continuum of care, ensuring optimum utilization of resources, service delivery, and compliance with external review agencies.
What You'll Do
- Coordinate clinical and/or psycho-social activities with the Interdisciplinary Team and Physicians.
- Monitor all areas of patients’ stay for effective care coordination and efficient care facilitation.
- Remain current regarding reimbursement modalities, community resources, case management, psychosocial and legal issues affecting patients and care providers.
- Appropriately refer high risk patients who would benefit from additional support.
- Serve as a patient advocate to maximize the patient’s and family’s ability to make informed decisions.
- Demonstrate knowledge of the principles of growth and development over the life span to provide age-appropriate care.
- Participate in interdisciplinary patient care rounds and conferences to review treatment goals, optimize resource utilization, provide family education, and identify post-hospital needs.
- Collaborate with the interdisciplinary care team, physicians, patients, families, post-acute providers, and payors in planning the patient’s care throughout the continuum.
What We're Looking For
- Graduate of an accredited program required for RN; or MSW/BSW with licensure as required by state regulations.
- Healthcare professional licensure required as Registered Nurse, or Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) if required by state regulations.
- Must read, write and speak fluent English.
- Must have good and regular attendance.
Nice to Have
- BSN preferred (for RN candidates).
- Certification in Case Management a plus.
- Two years of experience in a healthcare setting preferred.
- Prior experience in case management, utilization review, or discharge planning is preferred.
