Anew Behavioral Health, Ohio is looking for a Case Manager to provide comprehensive support and coordination of care for clients facing mental health challenges. You will assess client needs, develop individualized service plans, and collaborate with multidisciplinary teams to ensure the delivery of effective and client-centered services.
What You'll Do
- Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
- Manage case assignments, draft service plans, and review case progress regularly.
- Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
- Collaborate with therapists, clients, and client family members to ensure all behavioral health needs are met.
- Facilitate clients in achieving personal independence and assist in developing daily living skills.
- Uphold documentation and billing practices associated with CARF, CMS, and as training indicates.
What We're Looking For
- High school diploma or equivalent.
- NPI number and Ohio Medicaid ID.
- Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.
- Knowledge of behavioral health practices and community resources.
- Ability to work independently and as part of a team.
Nice to Have
- College degree in Human Services.
- CPR certification.
- A minimum of 3 years of experience in behavioral health or a related field.
Team & Environment
You will report directly to an Assigned Regional Director of Operations.
