Compass Career Solutions is hiring a Case Manager to assist individuals in identifying and acquiring supports to live independently in the community. This role uses a person-centered approach, focusing on coordinating services for vulnerable individuals within a mission-driven organization rooted in respect and inclusion.
What You'll Do
- Assist the person served in identifying and acquiring both natural and paid supports for independent community living.
- Function as an educator, resource, and service coordinator.
- Focus on coordinating services for vulnerable individuals and helping them navigate the complex service delivery system.
- Provide a systematic process of ongoing assessment, planning, referral, service coordination, monitoring, consultation, and advocacy.
- Directly coordinate a caseload and facilitate other services or supports.
- Have contact with a wide range of organizations, families, and community members.
- Act professionally at all times, representing the interests of the individual you support.
What We're Looking For
- A Bachelor's degree in social work, psychology, sociology, a closely related field, or be a licensed social worker.
- 1 year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.
- A valid driver’s license with an acceptable driving record and proof of automobile insurance.
- Ability to meet the qualifications of an unlimited driver position.
