Responsibilities
- Perform care consultations, service evaluations, and complete client support plans
- Conduct staff supervision sessions and client quality assurance checks
- Develop, update, and audit client care plans, and assist with digital care planning
- Maintain precise records for clients and caregivers using company software
- Facilitate introductions between clients and caregivers
- Foster and sustain positive relationships with prospective and current clients, their families, and other care professionals
- Provide guidance and mentorship to caregivers
- Review visit logs for accuracy and compliance
- Offer assistance to other members of the care team
- Keep in regular communication with clients and caregivers
- Carry out additional tasks as assigned by the line manager
- Take part in on-call responsibilities
