Support members with personalized care guidance by identifying and sharing appropriate provider options tailored to individual needs. You’ll ensure members receive timely access to care while adhering to service level expectations and data accuracy standards.
Key Responsibilities
- Engage directly with healthcare providers and facilities to source relevant, high-quality care options for members.
- Collaborate with Registered Nurses to navigate complex cases and ensure appropriate care routing.
- Identify emerging trends in member needs and provider performance, sharing insights to enhance service quality.
- Maintain up-to-date provider information by verifying and updating records regularly.
- Document all interactions and case details accurately within the ticketing system.
- Stay informed on evolving workflows, policies, and healthcare resources to support effective decision-making.
- Follow all regulatory and compliance requirements in daily operations.
- Complete additional tasks as needed to support team goals and member satisfaction.
Qualifications
Candidates should have at least one year of experience in care coordination within a healthcare environment, along with hands-on use of a Chromebook and Google Workspace. Prior experience in team-based settings is valued.
Work Environment
This role operates in a blended work model, supporting connection and collaboration across locations. Eligible candidates must reside in Arizona, Florida, Georgia, or Texas. The position uses Chromebook and Google Workspace as core tools.
Compensation & Benefits
Pay is $19.00 per hour. The role includes employee benefits and accrues vacation at a rate of 15 days annually. No equity is provided.
