Centene Management Company LLC is hiring a Care Manager (RN)

Centene Management Company LLC seeks a Care Manager (RN) to develop, assess, and facilitate complex care management activities for members with primarily physical health needs. You will be responsible for creating high-quality, cost-effective healthcare outcomes through personalized care plans and education. This is a remote position with a standard work schedule of 8am to 5pm PST.

What You'll Do

• Evaluate member needs, barriers to care, social determinants of health, and recommend or facilitate plans for the best outcomes.
• Develop ongoing care plans and collaborate with providers and community resources to address unmet needs.
• Identify problems or barriers to care and provide appropriate care management interventions.
• Coordinate between members, families, caregivers, and the care provider team to ensure adequate, person-centered care.
• Provide ongoing follow-up and monitoring of member status, condition changes, and progress towards goals, revising care plans as necessary.
• Provide resource support for local services based on assessments, such as employment, housing, or independent living.
• Facilitate care management and collaborate with providers and specialists to ensure timely access to care.
• Perform telephonic, digital, home, and other site outreach to assess member needs.
• Collect, document, and maintain all member information and care management activities for compliance.
• Provide and facilitate education to members and families on disease processes, care gaps, provider instructions, and benefits.
• Provide feedback to leadership on opportunities to improve care and quality delivery cost-effectively.

What We're Looking For

• A degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
• 2 – 4 years of related clinical or care management experience.
• Current RN - Registered Nurse state licensure and/or compact state licensure.
