CVS Health is looking for a Care Management Associate to join our Accordant Team. In this role, you will initiate outbound calls to patients and members living with complex medical conditions, explaining available care management programs and encouraging enrollment. You will be instrumental in educating members about health and wellness resources to improve their quality of life.
What You'll Do
- Work directly with patients and members to understand their complex medical conditions.
- Explain available programs that assist with care and improve quality of life.
- Initiate interaction with members through outbound calls.
- Educate members on health and wellness programs and encourage participation.
- Document calls while speaking to members, taking thorough notes.
- Help patients with rare health conditions understand specifics about health benefits eligible at no additional cost.
- Communicate features, advantages, and benefits of the program effectively.
- Represent the first impression of the organization to potential new members.
What We're Looking For
- 1+ years of experience in a medical or healthcare environment, inside sales role, or customer service/call center environment.
- 1+ years experience using Microsoft Office: Word, Excel, and Outlook—must be able to toggle between multiple systems.
- High School Diploma or GED.
- Must live in Pacific or Mountain Time Zone.
Nice to Have
- Experience in the healthcare industry.
- Specialty pharmacy background.
- Experience in an inside sales role.
