CircleLink Health is hiring a part-time Bilingual LPN Care Coach for its Medicare Chronic Care Management Program. In this remote role, you will be assigned a group of patients with multiple chronic conditions, providing monthly coaching to improve health outcomes through education, care coordination, and self-management support. The role demands high precision and accountability in a structured, performance-driven environment.
What You'll Do
- Utilize specialized care management software to call Medicare patients with 2 or more chronic conditions monthly.
- Build rapport and coach patients toward improved health using SMART goals and education on self-management.
- Implement and update the Plan of Care, including medications, appointments, biometrics, and interventions.
- Connect patients with community resources for transportation, personal care, prescription assistance, and social services.
- Conduct Transitional Care Management activities for high-risk patients discharged from hospitals to reduce readmissions.
- Close care gaps by encouraging preventive measures like annual well visits, vaccines, cancer screens, and follow-up appointments.
What We're Looking For
- Fluent in both English and Spanish.
- Current, unrestricted LPN Compact License or New Mexico LPN license.
- Proficiency with EPIC electronic health record and web-based applications.
- 3-5+ years of experience as a nurse.
- Self-directed, able to work independently while meeting performance metrics.
- Excellent organizational, time management, and strong telephonic communication skills.
- Strong critical thinking and problem-solving skills.
- Good with technology and eager to learn new software.
- Passion for nursing and improving patient outcomes.
Nice to Have
- Case Management or Chronic Disease Management experience.
- Certified Diabetes Educator credential.
- Experience with Motivational Interviewing or other behavior change communication techniques.

Benefits & Compensation
- $300 performance bonus after three months of successful employment.
- Compensation is paid per clinical encounter. Base rate is $11.00 per one, 20-minute Clinical Encounter per patient per month. Pay scales up with longer encounter times (e.g., $20.50 for 40 minutes, $29.50 for 60 minutes).
