Wider Circle is hiring a Bilingual Community Health Worker to run our Connect For Life program. As a trusted community member, you will serve as a vital link between members and health and social services, building trust and promoting wellness through group and one-on-one interactions.
What You'll Do
- Lead the Connect For Life program using approved curriculum and materials.
- Plan, promote, and host virtual and in-person events for members to foster relationships and support wellness outcomes.
- Regularly reach out to members by phone and in-person to check-in, identify needs, and encourage event participation.
- Conduct home visits to support members, provide health and social resources, and encourage participation in the Enhance Care Management program.
- Establish and maintain partnerships with local community-based organizations to bring additional resources to members.
- Record details of member interactions in the case management system using a computer, tablet, or smartphone.
- Achieve membership growth targets and engagement and retention goals by tracking attendance, feedback, and metrics.
- Identify natural leader members and invite them to serve as Connect For Life Ambassadors to assist in connecting with members and organizing meetings.
What We're Looking For
- Certified Community Health Worker.
- Enhanced Care Management or complex care management experience.
- 3+ years of relevant community outreach, teaching, facilitation, volunteer, or healthcare experience.
- Excellent presentation and communication skills, both written and verbal.
- Outgoing personality, positive attitude, and comfort engaging one-on-one and with groups in-person and via video/phone.
- Strong computer skills and ability to navigate web-based and app-based systems.
- Reliable transportation and comfort traveling to members’ homes.
- Willing to commute 20 - 40 miles.
- Flexible schedule and ability to work outside regular business hours.
- Commitment to a drug-free workplace and readiness for pre-employment substance abuse testing and background checks.
Team & Environment
You will work closely with a Case Manager as part of the member’s care team, reporting to an Area Manager.
