About the Role
The Clinical Nurse Navigator will guide patients through the healthcare system by offering personalized support, clinical assessments, and care planning, with an emphasis on improving access and health outcomes.
Responsibilities
- Conduct initial patient assessments to identify care needs
- Provide education on treatment plans and disease management
- Serve as a primary point of contact for patients and families
- Coordinate referrals to specialists and support services
- Monitor patient progress and adjust care plans as needed
- Document patient interactions and clinical updates in electronic records
- Assist with scheduling diagnostic tests and medical appointments
- Advocate for patient needs within the healthcare team
- Deliver culturally sensitive care in a bilingual environment
- Support patients in navigating insurance and financial resources
- Facilitate communication between providers and patients
- Promote adherence to prescribed therapies and follow-up visits
- Identify barriers to care and develop strategies to address them
- Participate in care coordination meetings with interdisciplinary teams
- Ensure compliance with clinical protocols and regulatory standards
- Respond to patient inquiries via phone, email, or in person
- Track patient outcomes and report on key performance indicators
- Assist in the development of patient education materials
- Contribute to quality improvement initiatives
- Maintain patient confidentiality in accordance with privacy regulations
