Salud Family Health is looking for a Behavioral Health Provider to join our integrated care team. In this role, you will provide behavioral health services to children, adolescents, adults, couples, and families, supporting them in self-management and behavior change.
What You'll Do
- Assess patients to determine the appropriate level of treatment in accordance with ethical and legal standards.
- Act as a patient advocate, ensuring vulnerable populations are fully informed and comfortable.
- Collaborate with Medical Providers, Dental Providers, other Behavioral Health staff, and all other team members to deliver high-standard integrated health care.
- Provide services appropriate to the age-specific needs of infant, child, adolescent, adult, and geriatric patients.
- Conduct mental health screenings using standardized instruments.
- Provide PCP-initiated consultation services including differential diagnosis, psychoeducation, brief intervention, and referrals.
- Consult with and advise other healthcare team members on assisting patients with social and emotional difficulties.
- Follow procedures for the electronic health records system.
- Manage psychosocial aspects of chronic and acute diseases.
- Provide psychotherapy to individuals.
What We're Looking For
- Master’s or doctoral degree in psychology, counseling, clinical social work, or equivalent.
- Must be licensed by the State of Colorado within 3 years from the date of hire.
- Fluency in Spanish and English is required.
- Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
- Ability to work effectively with other patient care team members.
- Ability to work through brief patient contacts and make quick, accurate assessments of mental and behavioral health conditions.
- Ability to collaborate with medical providers in the treatment of psychosocial issues.
- Sensitivity to low-income and ethnic minority communities is a must.
Nice to Have
- Prefer 2 years post-licensure treating patients with psychosocial issues.
Team & Environment
Works under the direct supervision of the Center Operations Director and the Vice President of Medical Services – Behavioral Health.

- Salary range: $64,000 to $82,000
