CVS Health is seeking an Analyst, Case Management to join our Special Needs Plan (SNP) care team. You will coordinate care for our Dual Eligible Special Needs Plan (DSNP) members by evaluating needs, addressing social determinants of health, and collaborating closely with care managers and social workers.
What You'll Do
- Conduct annual Health Risk Surveys to identify member needs for their Individual Plan of Care.
- Inform the assigned care manager of newly identified health, safety, or service risks.
- Complete delegated care coordination activities within established timeframes.
- Report any identified quality of care issues to the care manager and/or associate manager.
- Support member care coordination needs and drive solutions to address them.
- Use problem-solving skills to find alternative contact information for unreachable members.
- Employ motivational interviewing techniques to maximize member engagement and promote healthy lifestyle changes.
- Adhere to case management and quality processes compliant with regulatory, accreditation, and company policies.
- Meet performance metrics, including call volume, member engagement, and regulatory compliance.
- Navigate various healthcare technology tools to enhance care, streamline workflows, and maintain accurate records.
- Maintain strong collaborative relationships with members and colleagues.
- Communicate effectively, both verbally and in writing.
- Exhibit excellent customer service and engagement skills.
What We're Looking For
- 2+ years in behavioral health, social services, or a related field relevant to the program focus.
- Proficiency in the Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams).
- Access to a private, dedicated workspace.
- A High School Diploma with equivalent experience.
Nice to Have
- Case Management and Discharge Planning Experience.
- Managed Care Experience.
- An Associate’s or Bachelor’s Degree or non-licensed master’s level clinician in behavioral health or human services (psychology, social work, marriage and family therapy, counseling) or equivalent experience.

Team & Environment
You will be a member of the Aetna care management team, collaborating directly with a Care Manager, Social Worker, and other interdisciplinary team members, and reporting to an Associate Manager.
