Responsibilities
- Provide full-spectrum primary care to patients across all age groups
- Manage chronic conditions, perform preventive care, and address acute health issues
- Collaborate with physicians and clinical staff to ensure coordinated, high-quality care
- Maintain accurate and timely documentation in the electronic medical record
Requirements
- Licensed Nurse Practitioner or Physician Assistant in the state of Illinois
- Board certification in Family Medicine for NPs or appropriate PA certification
- Strong interpersonal and clinical skills
- Commitment to patient-centered care and health education
